"Open-Sky" Retropupillary Iris-Claw IOL Fixation

Drs. Meriem Ouederni and Fehmy Nafaa present a case of advanced bullous keratopathy associated with aphakia in a monophthalmic 76-year-old patient. After corneal trephination, synechiolysis was performed using an iris spatula, and an iris-claw IOL was placed onto the horizontal plane, centered on the pupil. An "open-sky" retropupillary fixation of the iris-claw IOL was then performed, as well as a pupilloplasty to treat the pupillary defect. Finally, the donor cornea was sutured with 16 separate 10-0 nylon sutures.
